California Retina Consultants proudly partners with Lumata Health to further support our patients through our Continuous Care Program. This program will be offered to patients beginning October 6.

About Our Continuous Care Program

Navigating a chronic eye condition can be overwhelming, but you don't have to face it alone. California Retina Consultants has teamed up with Lumata Health to offer comprehensive support for individuals with macular degeneration, diabetic retinopathy, and related conditions, in addition to the care provided by your retina specialist.

You’ll be assigned a Care Coach who will help you manage your eye condition and overall health. This support makes it easier for you to receive the right care at the right time, leading to better health outcomes.

Our Continuous Care Program doesn’t replace your current care, and we’ll remain your point of contact for any acute, emergent, or urgent needs. With support from Lumata Health and us, you can feel empowered in managing your eye health.

The Role of the Dedicated Care Coach

Your care coach is available to support you alongside your retina specialist by providing:

• Regular phone calls between office visits to check in on your vision and overall health

• Resources to help you manage your symptoms more effectively

• Assistance in finding prescriptions, vitamins, or other essential items for your vision health at the lowest cost

• Help with arranging transportation to your appointments, filling out paperwork, or refilling your prescriptions

• Support in understanding your doctor’s instructions and answering your questions

Continuous Care FAQs

• Who Is Eligible?

  Patients interested in the Lumata Continuous Care Program must meet the following criteria:

  • Be 65 years of age or older.

  • Have insurance coverage that results in little to no out-of-pocket costs, such as Medicare with a supplemental plan.

  • Have seen a participating Lumata provider within the past six months.

  • Have a qualifying chronic eye condition that requires specialized management and places the patient at significant risk of exacerbation, functional decline, or other serious complications.

• What Are the Costs Involved?

  This program typically incurs no costs if you have Medicare insurance with supplemental coverage or managed Medicare.

• What If I Feel Like I’m Doing Fine On My Own?

  Even if you feel stable in managing your eye condition, joining the Continuous Care Program can offer additional support and resources that may enhance your health outcomes and provide peace of mind.

• Is Lumata Health a Long-Term Commitment?

  Participation in the Lumata Continuous Care Program is not necessarily a long-term commitment. You can choose to engage as long as you feel it benefits your health journey. If at any point you decide you no longer need the support, you can opt out of the program.
